What are the services they offer?

  • Office visits, Mammograms, and Clinical Breast Exams, Pap tests, Prostate and Colorectal cancer screening tests recommended by your doctor.
  • They also offer Lung Cancer Screening tests, if recommended by your doctor and you meet the criteria, Health education and help with coordinating associated care

How to be eligible on their program?

  • You must be a Delaware resident.
  • Have health insurance that doesn't cover screenings.
  • Are aged 18 to 64 years old and not eligible for Medicaid.
  • Are ages 65 years old or older and do not qualify for Medicare.
  • Are not eligible for health insurance from the Health Insurance Marketplace.
  • Should have a household income between 139% and 250% of the Federal Poverty Level.
  • A household consists of the individual, their spouse, and all dependent children under 18 years of age.
  • Should have an annual household income between 139% and 250% of the Federal Poverty Level are eligible for Screening for Life if they are deemed ineligible for Medicaid.

  • And, not eligible for health insurance through the Health Insurance Marketplace

What is their insurance guidelines for underinsured?

  • The unmet deductible must be more than 15% of the annual income.
  • Insurance does not cover breast, cervical, colorectal, lung, and prostate screenings.
  • They will not reimburse for co-pays or co-insurance.
  • They do provide reimbursement for persons who have a high deductible relative to their income.
  • If your deductible is equal to or higher than 15% of your income, you may be eligible for SFL if you meet all the other eligibility requirements.
