Breast and cervical cancer screenings and heart health tests can save lives. This clinic is a participating provider for The Screening for Life program of Delaware. Screening for Life provides payment for cancer screening tests to qualified Delaware adults. The program is a cooperative effort of the Delaware Division of Public Health and the U.S. Centers for Disease Control and Prevention (CDC). Cancers can be treated more effectively if they are found early, and Screening for Life can help you get the tests you need at the recommended times. Services the Program Provide: Office visits Mammograms and clinical breast exams Pap tests Prostate cancer screening tests recommended by your doctor Colorectal cancer screening tests recommended by your doctor Lung cancer screening tests, if recommended by your doctor and you meet criteria Health education Help with coordinating associated care To be eligible for these FREE services, you must meet age, income and insurance guidelines. You may be eligible if you: Are a Delaware resident Have health insurance that doesn't cover screenings Are age 18-64 and not eligibile for Medicaid Are age 65 or older and do not qualify for Medicare Meet income guidelines Are not eligible for health insurance from the Health Insurance Marketplace Delaware Residents are eligible for Screening for Life if they have a household income between 139% and 250% of the Federal Poverty Level. The Federal Poverty Level is based on annual household income and household size. A household consists of the individual, their spouse, and all dependent children under 18 years of age. Delaware Residents with an annual household income between 139% and 250% of the Federal Poverty Level are eligible for Screening for Life if they are deemed ineligible for Medicaid. Enrollment is easy. Call the Screening for Life program 302-744-1040 . If you are eligible, you will be enrolled and mailed information on how to schedule your appointment with a participating health care provider. Screening for Life will help you get the recommended exams for your age and health history. Insurance Guidelines: Uninsured: No medical insurance nor private HMO Not eligible for Medicare or Medicaid Not eligible for health insurance through the Health Insurance Marketplace ” or ” Underinsured: Unmet deductible (deductible must be more than 15% of annual income) Insurance does not cover breast, cervical, colorectal, lung, and prostate screenings The SFL program does not reimburse for co-pays or co-insurances. The program does provide reimbursement for persons who have a high deductible relative to their income. If your deductible is equal to or higher than 15% of your income, you may be eligible for SFL ”if you meet all the other eligibility requirements.
